Hmmm. Tricky question. According to Unfinished Tales,

However, on the same page we also learn that Ar-Adûnakhôr

. . . was the first King to take the sceptre with a title in the Adûnaic tongue; though out of fear . . . a name in Quenya was inscribed in the scrolls.

So, following the logic of the question, I'd have to say that while Tar-Calmacil was the first King to be named by others(i.e. the King's Men) in Adûnaic, Ar-Adûnakhôr was the first King to voluntarily take his name in the Adûnaic language.

But OK... Here is the name : Dúnhere- Lord of Harrowdale. As it is explained in Note 13 in UT:
